Wellness/Immunization Policy
The School will notify parents if a child becomes seriously ill at school, especially if a child exhibits symptoms of a fever, nausea, or diarrhea. Students with such symptoms should not return to school until they have fully recovered and are symptom-free for 24 hours.

- Georgia Department of Human Resources School Certificate of Immunization, Form 3231
- Georgia Department of Human Resources School Certificate of Eye, Ear, and Dental Examination, Form 3300
- Birth Certificate
